Short answer: reishi (Ganoderma lucidum) has a genuine evidence base for immune modulation in humans and for sleep in animal models. The sleep research is largely preclinical, the immune research includes small human trials, and claims about reishi lowering cortisol in people are not well supported. This article separates those three tiers rather than blending them.

Reishi is the most extensively studied functional mushroom, which unfortunately makes it the one most often oversold. Below is what each domain actually rests on.

Polysaccharides and immune function

Reishi's beta-glucans — principally beta-1,3 and beta-1,6 glucans — bind pattern-recognition receptors on macrophages and dendritic cells, which is the proposed route to innate immune activation. This mechanism is well characterised in laboratory work.

In humans the evidence is real but early. A randomised trial published in Foods (2023) evaluated beta-1,3/1,6-D-glucan derived from Ganoderma lucidum in healthy adults and reported measurable immune-marker modulation. These are small, short studies in healthy people — not evidence of protection from illness, and we do not claim it.

It is worth being explicit about what the broader literature does not show. Systematic reviews of medicinal mushroom supplementation in clinical oncology, such as the review in Current Oncology Reports (2023), have repeatedly concluded that the evidence is insufficient to support these supplements as a treatment for any disease. Reishi is a wellbeing ingredient, not a therapy.

Triterpenes, GABA and sleep

Reishi's ganoderic acids — the bitter triterpenes — are the compound class behind most of its studied nervous-system activity, and the reason a water-only extract misses much of what reishi offers.

The most directly relevant sleep work is preclinical. A study in the Journal of Ethnopharmacology (2012) found Ganoderma lucidum extract prolonged total sleep time in rats, with the effect attenuated by a GABA-A antagonist — implicating GABAergic signalling. Later work in Scientific Reports (2021) described a second route, via the gut microbiota and serotonin metabolism.

Both are rodent studies. They establish a plausible mechanism; they are not the same as a human sleep trial, and reishi does not yet have one of the calibre that magnesium glycinate has. See Magnesium Glycinate for Sleep for the contrast.

Cortisol: where we will not overclaim

Reishi is frequently marketed as a cortisol-lowering adaptogen. We previously described an inhibitory effect on adrenal cortisol synthesis on this page; on review we could not substantiate that claim from the human literature, so we have removed it.

What is fair to say: reishi is traditionally used for calm, and there is preclinical evidence of autonomic modulation. Human trials measuring cortisol specifically are lacking. L-theanine: relaxation without sedation

L-theanine increases alpha-band brain-wave activity and modulates GABA and glutamate signalling. A randomised controlled trial in Nutrients (2019) administered L-theanine to healthy adults over four weeks and reported reductions in stress-related symptoms alongside improvements on some cognitive measures.

That is a single RCT in healthy adults, not a meta-analysis — an earlier version of this page described it as one, which was wrong. The practical point stands: L-theanine's profile is calm-but-alert rather than sedating, which suits the evening window without next-morning grogginess.

Anyone pregnant, breastfeeding, taking anticoagulants or immunosuppressants, or preparing for surgery should speak to a clinician first.
